目的 探讨揿针疗法对代谢相关脂肪性肝病(metabolic associated fatty liver disease, MAFLD)肥胖患儿身体成分与代谢健康指标的影响。方法 采用随机对照试验设计，纳入2024年1月—2025年1月在北京某三级甲等儿童专科医院，中西医结合儿童肝病门诊接受治疗的肥胖患儿100例。依据随机数字表法将患儿分为观察组和对照组，各50例。2组患儿均接受基础的健康宣教；观察组在健康宣教的基础上加用揿针疗法。分别于干预前及干预8周后，采集入组患儿的身体成分与代谢健康指标，包括体重、体脂率、腰臀比、基础代谢及BMI,以客观评价其干预效果。结果 干预8周后，2组干预前后组内比较，患儿体重、体脂率、腰臀比及BMI差异均有统计学意义(P<0.001)。2组干预后各项指标的组间比较除基础代谢(P=0.593)外变化程度明显，体重(P<0.001)、体脂率(P<0.001)、腰臀比(P<0.001)和BMI(P<0.001)差异均有统计学意义。结论 揿针疗法在MAFLD肥胖患儿的治疗中，能显著改善其身体成分及代谢健康指标，独特的疗效凸显了揿针疗法的优势，为MAFLD肥胖患儿的临床干预提供了一种安全、有效的中医外治疗法新策略。
Abstract：Objective To explore the effects of acupressure therapy on body composition and metabolic health indicators in obese children with metabolic associated fatty liver disease(MAFLD). Methods Adopted by a randomized controlled trial design, 100 obese pediatric patients with MAFLD who were treated at the integrated traditional Chinese and Western medicine pediatric liver disease outpatient department of a tertiary Grade A children's specialty hospital in Beijing from January 2024 to January 2025 were enrolled. According to the random number table method, the pediatric patients were divided into an experimental group and a control group, with 50 children in each group. Both groups received basic health education. In addition to the health education, the experimental group also received acupressure therapy. Before the intervention and after 8 weeks of intervention, the body composition and metabolic health indicators of the enrolled children, including weight, body fat percentage, waist-to-hip ratio, basal metabolic rate, and BMI, were collected to objectively evaluate the intervention effects. Results After an 8-week intervention, intra-group comparisons before and after the intervention showed no significant changes in body weight, body fat percentage, waist-to-hip ratio, basal metabolism rate, and BMI for the children in either group(P>0.05). Inter-group comparisons of the changes in these indicators before and after the intervention revealed significant differences, with body weight(P=0.011), body fat percentage(P=0.020), waist-to-hip ratio(P=0.007), basal metabolism(P=0.021), and BMI(P=0.025),which all showing statistical significance(P<0.05). Conclusion Acupressure therapy can significantly improve body composition and metabolic health indicators in obese children with MAFLD. It's unique therapeutic effects highlight the advantages of acupressure therapy, providing a new strategy of a safe and effective traditional Chinese medicine external therapy for the clinical intervention of obese children with MAFLD.
