证据金字塔的发展:从“6S”模型到“5S”模型

摘要:

医学领域新研究不断涌现,在海量数据中找到优质证据是循证实践的一大挑战。证据金字塔通过可视化的分级方法,帮助临床人员识别预先评估的证据,简化寻找和应用证据的过程。随着医学研究领域对循证证据理解的不断深入,证据金字塔也在持续演变和更新。本文介绍了2016年更新的证据“5S”金字塔模型,由原始研究、系统评价、循证指南推荐、综合性证据总结和决策支持系统5个层级构成。每个层级都系统性地从下层构建,使不同层级间的关系更清晰。证据“5S”金字塔模型从“自下而上”整合证据到“自上而下”应用证据的整合-应用双向模式,可为促进证据的高效检索和科学应用提供方法学基础,以及为循证护理提供系统化、分层级的证据资源框架,以支持临床决策和实践优化。

Abstract:

With the continuous emergence of new research in the healthcare field, identifying high-quality evidence amidst vast data has become a significant challenge for evidence-based practice. The evidence pyramid, through a hierarchical and visual approach, assists health professionals in recognizing pre-appraised evidence, thereby simplifying the process of evidence retrieval and application.As the understanding of evidence-based research in the medical field deepens, the evidence pyramid has undergone continuous evolution and refinement. This paper introduced the "5S" evidence pyramid model updated in 2016, which consisted of 5 levels: Original studies, systematic reviews, evidence-based guideline recommendation, comprehensive evidence summary and decision support system. Each level was systematically built from the lower level, making the relationship between the different levels clearer. The "5S" evidence-based pyramid model adopt an intergrated-application bidireational approach, combining "bottom-up" integration of evidence and "top-down" application of evidence could provide a methodological basis for promoting efficient retrieval and scientific application of evidence, as well as a systematic and hierarchical framework of evidence resources for evidence-based care to support clinical decision-making and practice optimization.
