目的 调研上海市老年慢性病患者的社会隔离与孤独感现况并全面分析其影响因素。方法 采用方便抽样法，于2022年6月—2023年2月，采用一般资料调查表、患者报告结局测量信息系统(patient-reported outcomes measurement information system summary table of social isolation, PROMIS)社会隔离简表8a、中文版的洛杉矶大学孤独量表简表(short-form of University of California at Los Angels Loneliness Scale, USL-8)、老年人社会支持评价量表(the social support appraisals scale, SS-A)、 PROMIS焦虑抑郁4a简表(PROMIS Short From-Anxiety/Depression 4a, PROMIS SF-Anxiety/Depression 4a)、电子健康素养量表(eHealth literacy scale, eHEALS),对上海市社区医院符合纳入标准的175例老年慢性病患者进行横断面研究。采用多元线性回归分析影响患者社会隔离与孤独感的独立因素。结果 上海市老年慢性病患者社会隔离平均得分为(12.68±4.950)分，孤独感平均得分为(13.87±4.218)分。多元线性回归分析结果显示：抑郁水平、对支持的利用程度、独居、评判能力是影响上海市老年慢性病患者社会隔离感的主要因素(P<0.05);焦虑水平、对支持的利用程度、照顾者数量、主观支持程度是影响上海市老年慢性病患者孤独感的主要因素(P<0.05)。结论 数字化时代下上海市老年慢性病患者感知的社会隔离感与孤独感高于健康人群且有逐渐加重趋势，应从多层面制定策略以有效疏解该群体的社会隔离与孤独感程度。

Objective To investigate and analyze the current status of social isolation and loneliness among elderly patients with chronic diseases in Shanghai, and to comprehensively analysis their potential influencing factors. Methods Using the convenience sampling method, a cross-sectional study of 175 elderly chronic disease patients who met the inclusion criteria in Shanghai's community hospitals from June 2022 to February 2023 was conducted using the general information questionnaire, the patient-reported outcomes measurement information system summary table of social isolation(PROMIS) social isolation scale 8a, the short-form of University of California at Los Angels loneliness scale(ULS-8), the social support appraisals scale(SS-A),the PROMIS short from-anxiety/depression 4a(PROMIS SF-Anxiety/Depression 4a),and the eHealth literacy scale(eHEALS). Multiple linear regression was used to analysis the independent factors affecting patients' social isolation and loneliness. Results The mean score of social isolation and loneliness for elderly chronic disease patients in Shanghai was(12.68±4.950) points and(13.87±4.218) points, respectively. Multiple linear regression analyses showed that depression level, level of utilization of support, living alone, and judgmental skills were the main factors affecting the sense of social isolation of the elderly chronic disease patients in Shanghai(P<0.05); Level of anxiety, level of utilization of support, number of caregivers, and subjective support were the main factors affecting the sense of loneliness of elderly chronic disease patients in Shanghaii(P<0.05). Conclusion The perceived social isolation and loneliness of elderly chronic disease patients in Shanghai in the digital era is higher than those of the healthy population and has a tendency to worsen gradually. Strategies should be formulated from a multidimensional level to effectively alleviate the social isolation and loneliness level of the elderly chronic disease patient group in Shanghai.
