基于能岗匹配理论院前急救护士核心素养评价量表的编制及信效度检验

摘要:

目的 构建院前急救护士核心素养评价量表并进行信效度检验。方法 基于能岗匹配理论,采用文献回顾、半结构式访谈,初步拟定院前急救护士核心素养评价量表条目池;通过Delphi专家函询、层次分析法及预调查,形成初始量表。于2023年10—11月,采用方便抽样法选取山东省16个市(区)53所三级甲等综合医院的院前急救护士作为调查对象,进行量表的项目分析、信效度检验及K-均值聚类划分等级。结果 回收有效问卷576份,回收率96%。院前急救护士核心素养评价量表包括3个维度、49个条目。2轮函询专家积极度均为100%;专家权威系数分别为0.788、0.806;专家肯德尔和谐系数分别为0.188和0.221(均P<0.05)。量表水平的内容效度指数为0.987,探索性因子分析提取10个公因子,累计方差贡献率为83.701%;验证性因子分析结果显示模型拟合较好;条目水平的内容效度指数为0.930~1.000;量表总的Cronbach′s α系数为0.959,折半信度为0.838,重测信度为0.999。K-均值聚类分析法显示,低水平为0~2.19分;中等水平为2.20~3.28分;高水平为3.29~4.00分。结论 院前急救护士核心素养评价量表具有良好的信效度,能客观地评价院前急救护士核心素养水平,对院前急救护士的准入、培训及考核具有一定的借鉴、指导意义。

Abstract:

Objective To construct the core literacy evaluation scale of pre-hospital emergency nurses and conduct its reliability and validity test. Methods Based on the theory of ability and post matching, literature review and semi-structured interviews were used to preliminarily develop a pool of items for the evaluation of core competencies of pre-hospital emergency nurses; The initial scale was formed through Delphi expert inquiry, analytic hierarchy process, and pre-survey. From October to November 2023, a convenient sampling method was used to select pre-hospital emergency nurses from 53 tertiary Grade A comprehensive hospitals in 16 cities(districts) in Shandong province as the survey subjects for item analysis, reliability and validity test, and K-means clustering classification of the scale. Results A total of 576 valid questionnaires were collected, with a response rate of 96%. The core literacy evaluation scale of pre-hospital emergency nurses included 3 dimensions and 49 items. The enthusiasm of 2 rounds of inquiry experts were 100%; The expert authority coefficients were 0.788 and 0.806, respectively; The Kendall harmony coefficients of experts were 0.188 and 0.221, respectively(both P<0.05). The average content validity index of the scale was 0.987, and exploratory factor analysis extracted 10 common factors, and the cumulative variance contribution rate was 83.701%; The results of confirmatory factor analysis showed that the model fitted well. The content validity index for the entries ranged from 0.930 to 1.000, and the total Cronbach′s α coefficient of the scale was 0.959, with a half reliability of 0.838 and a retest reliability of 0.999. The K-means clustering analysis method showed that the low level was 0-2.19 points; The average score was 2.20-3.28 points; The high level was 3.29-4.00 points. Conclusion The core literacy evaluation scale of pre-hospital emergency nurses has good reliability and validity, which can objectively evaluate the core literacy level of pre-hospital emergency nurses, and has certain reference and guidance significance for the access, training, and assessment of pre-hospital emergency nurses.
